I can't think of any reason why payment of physician services should be lumped into the same pool of money that pays for your lab or your MRI's, or your blood transfusion. I can't think of any reason why paying Dr Smith out of your own pocket should preclude you from enjoying the benefits of your monthly premium paid toward Part B. It is simply ludicrous that the systems is set up like this. It has destroyed access for the poorest people.

Practically nothing about Medicare makes sense, and since it was created and is run by the government, it makes me tremble in my shoes to think there are so many people advocating national health insurance sponsored and run by the this same US government.
